Appointments & Office Hours
The office is open Monday through Friday from 8:00am to 4:30pm,
but each doctor has a separate weekly schedule of appointments, and patients are seen by appointment only. To schedule an appointment, or to change or cancel an appointment, call (312) 470 8990. For your first appointment, be sure to arrive at the office 30 minutes prior to your appointment to allow time to complete registration forms.
All patients should
bring their health insurance card, another ID card [with photo], and a list of your medications and supplements to every office visit. Also please inform us of any change of address, phone number, or insurance.
If lab studies may be necessary, fasting is recommended (no food after midnight; you may have black coffee, unsweetened tea, or water). Please take your usual medications that day.
Failure to notify us of an appointment cancellation at least one day prior to your appointment will result in a charge to your credit card.
Spending time with a patient in order to obtain detailed medical history information is a key element in the art and science of Internal Medicine, and all of the physicians at our office are experienced in this aspect of your visit. However, some patients have difficulty recalling all elements of their history, and having extra time to remember various previous illness and symptoms in advance of your visit may be helpful. You may wish to download this optional
health history form, and fill it out to bring to your appointment. You and your doctor may find it useful in supplementing the information obtained during your visit.
Please note that minors [children under the age of 18] are not typically treated at our office, but when such a patient has been accepted by a physician here, consent for treatment must be signed by the patient’s legal guardian before the patient is seen. The consent form can be downloaded
here, filled out and signed, and brought to the appointment.
